Good Faith Estimate

As part of the Federal "No Surprises Act" (2020), healthcare providers must furnish uninsured/ self-pay patients with a Good Faith Estimate of costs when inquiring about services or prior to scheduling a service.  

I will provide this estimate at the time of inquiry for services, or if we schedule an appointment.

Fees & Insurance Participation 


$190 usual-and-customary hourly rate for most services (effective 1/1/2023)

Need-based graduated fee schedule available

In-Network Provider:

• Aetna
• Anthem-Blue Cross of California
• Blue Shield of California (including Blue Shield Covered California plans)
• Magellan
• Optum / UnitedHealth 

Contracted Provider:

• Kaiser of Northern California (pre-authorization required) 

If you wish to seek out-of-network reimbursement from another insurer, I will provide you with a statement of services with applicable procedure codes and fees.
